how long have you been on this? fatigue is normal at first but sudden severe fatigue after a while isn't. get blood panels done because fatigue can be tons of different things. shouldn't be getting medical advice on reddit.
People do it both ways—personal call. Some use stated mg, some adjust to tested amount. But testing one vial isn't enough data for the whole batch. Mass and purity at least tell you what's in that one. I just dose by stated mg and count overfill as a bonus 😂
