Call someone qualified — not us — for medical help

If you think you are having a medical emergency, including but not limited to:

  • Severe abdominal pain (especially radiating to the back)
  • Vomiting that won't stop or contains blood
  • Black or bloody stools
  • Chest pain, difficulty breathing, or fainting
  • Sudden severe headache or sudden vision changes
  • Signs of low blood sugar (confusion, sweating, fast heartbeat) if you are on other diabetes medications
  • Any symptom that feels urgent to you

Call your local emergency number (911 in the US), your prescriber, or go to an emergency department immediately. Do not post on the forum asking what to do.

GLP-1 medications carry specific risks

GLP-1 receptor agonist medications carry serious warnings on their FDA-approved labels, including but not limited to:

  • Boxed warning for thyroid C-cell tumors (medullary thyroid carcinoma) — contraindicated in people with a personal or family history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Pancreatitis — stop the medication and seek medical attention if you develop severe, persistent abdominal pain.
  • Gallbladder disease including gallstones and cholecystitis — may require surgery.
  • Acute kidney injury — particularly in the setting of severe vomiting, diarrhea, or dehydration.
  • Severe gastrointestinal side effects, including gastroparesis — FDA updated labeling in 2024-2025 to reflect these risks.
  • Possible increased risk of suicidal ideation — the FDA and EMA have both evaluated this signal; discuss with your prescriber if you have a history of depression.
  • Diabetic retinopathy worsening in patients with pre-existing diabetes.
  • NAION (non-arteritic anterior ischemic optic neuropathy) — a signal identified in 2024 for semaglutide; sudden vision changes require urgent evaluation.
  • Contraindication in pregnancy — GLP-1s should generally be stopped 2 months before a planned pregnancy.

Read the FDA-approved prescribing information for your specific medication, and discuss its risks with your prescriber.
