I began using a semaglutide medication around mid-January. Began with 0.2mg, and the first month was incredible. Zero cravings, less inflammation, and I shed about 10 pounds in the initial 30 days. Since it was effective, I opted not to increase the dose, consulting with my online provider to keep it the same. Around week six, I decided to try an increase to 0.25mg, but it didn't do much, and if anything, I felt hungrier. I then caught a nasty bug and was taking a lot of medication for it, so I skipped a dose. Last week, I tried 0.325mg.

For the first few days, I felt great and had fewer cravings, but by earlier this week, they were returning. My next dose is coming up, and I'm unsure what to do. I discontinued my online membership, but I still have plenty of medication. So, I can't seek advice through that platform. I'm planning to get prescriptions from a local nurse practitioner to save money, so I didn't want to pay for more months with the online place when I didn't want to get my prescription that way any more.

I'm not too worried about losing weight; I just really loved having no cravings and I want to get back to that if possible. I'm prediabetic, so it really helped with my sugar cravings. I've always focused on sufficient protein and fiber, and I've been careful to ensure I'm getting enough calories. Any advice would be greatly appreciated!

I stopped sema maybe 3 months ago, and was on it for a year. I had a 2 month backup supply of doses in my fridge that I saved up. I used those extra doses twice a week, maybe Tuesdays and Fridays.

When I realized the effects weren't as strong as I preferred, I just took a bigger dose. I'm not suggesting you do this, but that's what I did. If I was supposed to have 45 units (on my syringe), the next dose I'd increase to 50 units.

Trust your body and eat well. Protein, protein, protein. Avoid soda and sugary stuff.

I prepared myself, knowing I would eventually have to stop the injections. I adopted healthy eating habits and routines. Even though it's been a few weeks without a dose, I'm still doing great with my healthy eating habits. I am still losing weight without even trying.

The reason to increase your dose is med tolerance, not merely weight reduction.

You should stay ahead of the cravings during the titration phase, not lag behind them. You're playing catch-up now. You'll get there, but remember the core rule: titrate due to tolerance, not only for more weight loss.
 
It's pretty normal for food cravings to reappear while you're finding the right dose, especially early on. A lot of people find that lower doses are very effective at first and then their body adapts, so the appetite suppression isn't as noticeable until they increase the dose. Being sick and skipping a week might have messed with your body's rhythm, too.

Since you're planning to see a local NP, it might be worth discussing the dosing schedule and whether it's a good idea to gradually increase from where you currently are. I've also heard of people including smaller habits that support appetite or blood sugar control while figuring out dosing. Some people mention TrimRX, but most people view it as supportive alongside their routine rather than the primary fix.

I get that, @Robin38. It's great when the food thoughts quiet down. Focus on the basics, like sleep and water intake. Those def affect my cravings.
 
Never had the stomach issues at all. It just killed the constant thinking about food, and honestly the urge to drink went away too. Real side effect though has been the anhedonia, but it's gotten way better since I hit the gym regularly.
 
Foods that break down easier help, especially those with natural enzymes like pineapple or papaya. Cleaner eating overall tends to cut down on that symptom too.
 
Food noise returning after a quiet period hits harder than it would otherwise - the contrast makes it worse. First check: are you due for a dose adjustment? Sometimes return signals you've adapted to the current level. Stress, sleep, and hormonal timing affect it week to week too. Most people find it responds to a dose bump or just riding out a rough stretch.
 
Got to 185 then crept back to 205 on the same dose, felt pretty defeated. Started at 315, began treatment at 297. The food noise is brutal but I'm not bailing on this.
 
We tried it together too. The refrigerated pens from Mexico seemed like a hassle to bring back, so we got the tablets instead. Hated the fasting requirement honestly. When we switched to injections, I ended up keeping most of them since I had better results. He's probably getting better suppression than he thinks, but won't admit the appetite shift is real.

Cagrilinide targets the amylin pathway, which addresses food noise from a different angle than GLP-1 alone. The pairing is worth trying if sema alone isn't holding.
 
First three days at the starter dose is effectively zero - the therapeutic level hasn't built yet and the appetite signal usually doesn't change until week 2-3 at the earliest.
