been on mounjaro 2 years - started 296, now 195 at 42f. stuck below 190. doc added 500mg metformin xr once daily to help. took first pill yesterday + shot today. anyone else using this combo?

Three months to a full A1C point is the result 5mg is calibrated for.

The radiologist-ordering-labs workaround is creative but ultimately not sustainable as a primary care structure - a good endocrinologist would transform the A1C trajectory work you are already doing. Worth putting real effort into finding one who actually manages metabolic disease rather than just reviewing results.

Yeah, compound sema maxing out at 2.5 and then switching to compound tirz at 10 and titrating up is a pretty standard progression when sema hits its ceiling. The OP's A1C drop in three months on sema is the direct result of what happens to glucose when appetite suppression reduces intake consistently. What's the A1C story been since the tirz switch?

the triglyceride drop tends to be the one that surprises people most - the A1C improvement is expected but the lipid panel coming in clean alongside it is a real metabolic bonus. visceral fat is the one that does most of the damage so that reduction is the one worth tracking long term

the 'articulate medical necessity' process for lab coverage is unevenly applied - standard metabolic panels and A1C get through most prior auth processes with a standard T2D or pre-diabetes code, but anything outside the routine panel requires documented clinical reasoning. tumor markers are specifically flagged for high out-of-pocket rates because they require a diagnosis code that most primary care offices aren't coding proactively

That's so great, really happy for you! Appreciate the encouragement. Getting by on protein shakes, the occasional bite of grilled chicken, and string cheese here and there.
Also dealing with type 2, my A1C sat at 9.1 last checkup, which is why my doctor added insulin. Hoping all this effort actually pays off eventually. Not exactly loving feeling this rough though.
