45 grams of fiber a day is significant. Real food sources work better than supplements for most - oats and chia are slow-digesting and keep things moving. The hydration piece matters more than people realize. Magnesium is another lever if fiber and water alone are not enough.
 
The glucose spike nausea is a separate mechanism from the GI motility slowdown causing the constipation - addressing both at once usually means finding a soluble fiber that works without fermentation byproducts, plus a stimulant laxative like bisacodyl on the days you're most blocked, rather than just increasing bulk fiber.
 
The painkiller plus GLP-1 combination is particularly rough on constipation - opioids slow gut motility through a different mechanism and the effects compound rather than cancel. The fiber plus probiotic shake approach is sound; consistent morning timing rather than reacting to symptoms tends to work better.

Never dealt with constipation on ozempic myself, but I was already eating plenty of fruit, veggies, and beans, so my fiber intake was already well above target. My advice would be to push the fiber higher and stay well hydrated.
