Faster Sema Dose Increases?

I've been looking at the published trials, and the typical ramp-up seems to be 2mg every month, then 4mg, then 8mg. Some people even started at 4mg or 8mg, and it mainly came down to how they handled the side effects. I've only had one 2mg injection so far, and I barely felt anything. I'm thinking about pushing it a bit faster, like 2mg for one shot, then 4mg for three shots, then 8mg for two shots, and settling at 8mg long-term. Anyone tried something similar? Or have any thoughts on speeding up the titration?

That's almost exactly what I did! Started at 2mg, then 4mg for a little, then 8mg. I'm staying at 8mg for now. From what I read, you don't get as big of a benefit jumping from 8mg to 12mg as you do going from 4mg to 8mg.

I agree

I think the side effects at 12mg aren't worth the minor extra weight loss. I'm hoping 8mg is the sweet spot for me.

I did a different titration schedule. I did 2mg for a couple of weeks, 4mg for three weeks, and then bumped up to 6mg last Monday. I've lost about 20 pounds so far! I can still eat, but I don't really want to.

I think it really varies person-to-person. I've seen some people have a huge initial drop in weight, which is often just water weight from reduced inflammation. Then it slows down. I don't even look at the scale. My doctor monitors my bloodwork to see if it's working.

For those who are going on vacation and might miss a dose: The half-life of these medications is fairly long (several days). Missing one dose isn't usually a problem, but contact your prescribing physician for guidance if you're going to miss multiple doses.

I'm on my fourth 2.5mg shot, and I've barely had any side effects – some mild nausea on shot days and a little constipation. I am wondering: should I increase to 5mg after this one?

I stayed on 2.5mg until it stopped working! I was on it for 9 weeks, then moved up to 5mg. Now, a year later I am still on 5mg, so I will be tapering off of it soon, as I am nearly at my goal weight. What does your doctor advise, @Care_Wine?

Heart rate goes up with higher doses, peaks around 6 months, then gradually settles back down though still a bit elevated from baseline.
