Gaining back weight on sema/triz?

Stalling before reaching 6mg and weight holding flat while knowing calories should be tracked more closely is the version of the plateau where the behavioral piece is the lever. 185 to 169 and holding is not a regain situation yet - it's a 'how do I keep the momentum going' situation. Tracking for even two weeks tends to show exactly where the intake is landing relative to where it needs to be.
 
Titrating up on sema before switching to tirz is worth trying if you are not at the max dose yet. The regain pattern often reflects a dose that stopped working rather than the medication failing completely. The eating habits question matters - what has changed in the tracking since the early months when the loss was happening?
 
23 lbs in 3 weeks is on the high end but the water and inflammation explanation is actually the most likely factor for people who started with insulin resistance. 185 to 169 in the OP's timeframe is real movement. The question about gaining back is usually about whether the dose is still active or whether something changed in the diet side - how does the appetite suppression feel now compared to early on?
 
Been cycling through sema, tirz, and reta for three years now and I'm only trending upward. Dropped 24 lbs that first year on sema, held steady on tirz for a stretch after that, tried switching to reta and gained 10, went back to tirz and gained even more, now I'm back on sema, lost 5 and then put 10 back on. Eating two prepackaged metabolic-focused meals daily plus one coffee. Working out three to four days a week. Genuinely not sure what to try next!
 
Tirzepatide works differently than semaglutide, but I wish someone had told me upfront it doesn't click for everybody right away. Genuinely thought it was some kind of miracle. My life's always been pretty disciplined around food and exercise. Turns out I just kept overeating regardless, my mind just wouldn't settle down until I'd eaten past the point of comfort, even with genuinely healthy food. Given that, results didn't come quickly for me. Didn't have much inflammation to shed and barely changed my eating since it was already solid. Took a long while to realize all this medication really accomplished was making me feel full a bit sooner, and I had to consciously pay attention to that cue myself. At first I just let my body do whatever it wanted since so many people here described losing tons of weight without even trying. Nope, not my experience. Actually gained at first. Six months into this and I'm realizing the overeating pattern in my head is the real problem here, not my everyday habits. I need to retrain myself to stop eating once I'm satisfied instead of stuffed. This medication hasn't touched that piece at all. Going to have to handle that mental work myself.
 
Please ease into things. Pull back on carbs and step on the scale less. A pot you keep staring at (your belly) will never seem to boil. The pounds come off once you shift your attention elsewhere.

I stayed at 15mg of the third compound for eight months before a serious accident forced me to stop abruptly. Looking back, I think I was rushing toward 15 just to chase the biggest possible result. During my recovery period I regained close to 60 lbs. Wake-up call, this is a lifelong medication! I restarted at 5mg and the results came back strong! Spent roughly three months there and the pounds started dropping again. Bumped to 7.5 a month back and it's still working well! I'm within 3 lbs of where I was at my lowest ever on this drug. My honest take, there's no reason to rush toward 15mg, our bodies will let go of weight without needing constant obsession.
