GLP-1 cost analysis?

Hong Kong Yanpep International
Batch reconstituting and freezing aliquots is one of the most underrated cost controls - the per-dose math improves significantly when you're not losing product to partial vials.

Going back to Sema because it was working and the cost is manageable is a completely rational move. The peer pressure toward Tirz is real here even when Sema is doing the job. There's no prize for using the more expensive or newer option if the results are already there.

Sema's basically the OG at this point, everyone calls her first-generation glp1 these days... still, I hang onto her and she puts up with me well enough. I believe the newer trial data tops out around 7.2mg, so once tirz or reta maxes out and hunger's still gnawing at me, I'll probably have to accept starting sema back low at 1mg... and yeah, I'll absolutely be tempted to pin three more the very next day

Not great with Reddit myself, but search Embody Glp1 on Google and you'll find a post from the tirz subreddit with more detail. Sounds like the price climbs after your first round of dosing.

Been stuck in basically the same spot myself, just for a longer stretch. Held steady for the better part of a year. Wish it counted as maintenance already, but there's still more I want to lose. Finally decided to move up a step but couldn't track down anywhere able to fill that particular amount. No insurance coverage here, so staying in close contact with my regular doctor to make sure care stays solid. He was hesitant about switching me over since his research suggested restarting from the lowest dose. That's actually not accurate, doctors more plugged into this drug class tend to move patients over at a higher starting point instead. A little nervous about adjusting to something new though, the first medication genuinely wrecked me for the first several months.

Heard that particular amount doesn't carry people very far anyway, so decided to go with the advice I'd been given. Starting the new medication this week at a solid opening dose.

Same thing happened when I moved from sema straight to tirz, no gap between them. Started at the lowest amount, only up to 5 currently. Haven't noticed any results yet.

Odd that so many drugs in the article deliver small percent losses (10-15%). How do they expect these to compete? Probably price, otherwise why would patients skip Sema, Tirz or Reta? Nice find.

Back when I used compounded tirz, cost worried me a lot. I've spent a good chunk of my life broke, counting every penny, and those scarcity fears stuck around. It backfired: I kept my dose low even though I knew I could and should go up, and stalled a whole month. Thankfully my situation has since changed so price no longer dictates my dosing. If it helps, the 4 week guideline doesn't matter to me either. At first I did, and it's sensible for people who follow it, though these days I go by a 1%-2% flow, roughly half your pace. When 4 weeks pass with under 2% of my bodyweight gone, or 2 weeks pass with under 1%, I add 0.5mg to the dose.

Going with compounded tirz made me so happy, even at $400 a month it was doable. Then I found cheaper deals, read the hints of gray area on Reddit, and wanted in. I never expected all the extras: heart and maybe brain benefits, and lots more in studies. It honestly feels like a wonder drug. My skin is much clearer since starting tirz, five months in, and the dark spots are fading, maybe from less inflammation and better food choices, or from less compulsive picking, which I did since childhood and now do at maybe 10-20% of before. I'm down to maybe 4 to 6 beers monthly, from 6 to 12 weekly, and I focus better without my ADHD meds every day (still take them, just not daily. Don't stop anything prescribed, this is only my own reading).

I'm only at .5mg so far but doing decently, down 15 lbs after six weeks, so I'm HOPING 1mg keeps working long term. Should I need a pair of pens monthly, I could likely swing it, though it's pricey for me. Hoping prices keep falling, or higher-dose generics get approved, before higher doses become my problem.

Cost is the wrong variable, and I think that is why so many people stall on it.

The number that matters is not your monthly spend. It is your cost per milligram, because the monthly spend only rises when you raise the dose, while the $/mg is what you can actually control.

A concrete example using our own catalogue, because it is the only set of numbers I can vouch for:

Tirzepatide, per vial, shipping not included, single-vial price:
5mg $48 -> $9.60/mg
10mg $67 -> $6.70/mg
30mg $134 -> $4.47/mg
60mg $236 -> $3.93/mg

Same peptide, same laboratory, same certificate format. The 5mg vial costs 2.4x what the 60mg costs per milligram. Nothing about the molecule changed. What changed is that somebody had to fill, cap, label, pack and ship a vial for 5mg of powder, and that fixed cost gets divided by five instead of sixty.

So the practical answer to "should I go up to 1mg" is: at 1mg a week you use about 4.3mg a month. That is $29 a month at the 10mg price and $17 a month at the 60mg price. Going from 0.5mg to 1mg costs you roughly fifteen dollars a month in real terms, not double your bill. The doubling feeling comes from buying the same small vial more often.

Two things that are worth checking before anyone quotes you a $/mg figure, including mine:

First, $/mg assumes the vial contains what the label says. We have a tirzepatide certificate on file that came back at 94.95% purity with 14.15mg measured in a vial labelled 10mg. If you divide price by the label, that vial looks like the best deal on the page. If you divide by the measured content, it is 1.4x cheaper than it appears and also not a clean product. Ask for measured content, not just purity percentage.

Second, the 10mg vial is the worst value per milligram in almost every compound, and it is exactly the size people start with because they are not committed yet. That is not a conspiracy, it is fixed cost per vial, but it does mean the first purchase is usually the most expensive one per mg you will ever make.

Source of the prices above is my own list, so treat it as one data point rather than a market rate. If anyone wants to post theirs in the same format (compound, vial mg, price per vial, shipping in or not, measured mg if you have a certificate) I will keep a table and post it back. I would rather have five honest lines than nobody's opinion.
