GLP-1 Titration: How often y'all upping?

Hey everyone! I'm curious about your experiences with increasing your GLP-1 dosage. What factors do you consider when deciding to move up, and how often are you adjusting your dose?

Personally, I'm currently injecting every 5 days and seeing decent results. I'm torn between increasing based on my bi-weekly weight loss average or the level of appetite suppression I'm feeling. It's tough to balance the science with intuition!

I started mid-May, did a fortnight at 2.5, then moved to 5 and stayed there. I recently started splitting the dose -- Sunday and Wednesday. TBH, I am not sure it makes a difference. The initial appetite suppression has lessened, but I'm not going to amp up just to chase that feeling. I track my weight daily and it consistently trends downward (except on vacay where I added maybe 0.1lbs, lol). I worry about building tolerance. What happens when you get to the highest dose and it quits working? I don't want to be there. I'm trying to stick to the lowest effective dose. Plus, you gotta think about $$$. Is it worth it to spend more for faster results? I'm also advanced in age, and trying to drop 80+ lbs this time, so I'm pacing myself. So far so good at ~2 lbs per week. I'm almost at the halfway point, and am hoping my skin will be ok. I still get discouraged when the scale goes up -- I will think "gonna go to 7.5!" (or maybe 6 b/c even #'s are better!). But I wait and see if I weigh less on Sunday, and check my measurements... so far I'm still trending down.

I went to a 5-day schedule while on Mounjaro, after reading online about the half-life of tirzepatide. I was on 2.5mg for 2 months & lost 20 lbs, then went to 5mg. Since then, I've been going up every month as the appetite suppression fades and the food noise comes back. I'd prefer to stay on the lowest dose that works, but I don't want to stall by trying to stretch each vial too much.

Everyone's different, so experiment to see what works for you. The recommended schedule is based on trials, primarily with diabetics, but the weight loss schedule is similar. I just don't want to risk stalling by making small increases and not getting the results I want.

Based on trials and research, many dosing protocols prioritize medication adherence. For example, while daily tirzepatide administration might be optimal based on the half-life, pharma dosing is weekly because this has the highest adherence rate. The higher dropout rate in trials may be due to GI discomfort. Is weekly dosing optimal? I doubt it. But it's easier for most people. I personally can adhere to a non-weekly schedule, and that's what I'm going with.

I started at 1.25mg every 5 days. After 8 doses, I upped it to 1.66mg every 5 days. It's harder to evaluate the effects because I took extra days between doses twice to reduce ISRs and for bloodwork. I don't plan on the 100% dose increase that the pharma schedule uses, because those jumps seem to cause the worst side effects. I don't want to lose weight quickly, so I can increase dosage more slowly. It's not a race to 15mg weekly, especially with the cost. At the end of the day, we are all n=1 making our way through this, trying to figure out what works best for each individual.

FYI, Tirzepatide acts on both GIP and GLP-1 receptors. Semaglutide only acts on GLP-1. That's why some say it's more effective.

Thanks @Fit-Road! Good to know that Tirz has a dual mechanism. Makes sense why people are getting such good results!

Staying consistent with your weight or slowing down loss should make it easier putting on muscle compared to losing it. Upping your protein helps a lot, though it would help knowing what you eat normally now.
