Hunger's Back?!

Hong Kong Yanpep International
Backing down when hunger returns is the right instinct rather than pushing through. The 2.5-3.5 range is where most people find stable appetite suppression without the side effect load of higher doses.

Hunger returning at a year usually means receptor adaptation - most people adjust dose or interval at that point.

100 lbs down with 18-20 to go is close enough to goal that the hunger returning may actually be the body defending the lower set point more aggressively - the final stretch often needs a dose recalibration to get the signal back to where it was in the first 6 months.

A few weeks at a higher dose can shift how 5mg feels - the recalibration period usually settles within 3-4 weeks of staying consistent.

The medication continuing to provide value beyond appetite suppression - metabolic markers, blood sugar stability, inflammation - is why some people keep taking it even when the hunger effect fades. The 'still taking it because' is a reasonable calculation depending on what those other markers look like.

8 lbs from goal and already planning the maintenance dose is exactly the right mindset!! Tirz 5mg stepping down gradually is the smart approach. So close to the finish line - this is going to be an amazing update post when you get there!!

Food noise returning while still getting full quickly is the specific pattern that points to dose tolerance rather than complete attenuation - the satiety signal is there but the appetite signal is breaking through. That's the signal to have a dose increase conversation rather than managing through habits alone.

2.5mg wearing off at 5-6 weeks and hunger creeping back is almost exactly what the OP describes. the move to 5mg solving it is the go-to response when that happens. it is not the medication failing, it is the dose needing to match the current state. pretty encouraging that it worked cleanly.

Right there with you. Lost the same amount too, currently at a solid amount. Stopped because of cost and gained some back. Now back on it and it feels less effective this time around. Started small and moved up after a few weeks. Planning to move up again since it doesn't feel like it's working as well anymore. Remember a certain amount really knocking things down before, but now at that same level hunger and food noise are both still there.

Your appetite and food noise absolutely come back, they do not vanish forever. Six months on 10 mg for me. Some weeks I cannot look at food, other weeks I cannot stop eating. My fix was to eat more, but only when truly hungry, after checking first. Then plates get loaded with vegetables and plenty of protein, plus fiber, hydration and fruit. I keep losing without constantly fighting hunger. Hope this helps!

Year one gave me 50 lbs off; year two, on 2 mg, I'm creeping back up. My appetite has returned too, though milder than before. I wish I had something more uplifting to say, just know you're not alone!

Same, I'm hungry and only lose if I diet. Over 2 years in on 2mg a week. When I do eat, big portions are impossible, so I guess that's something.

Shannon_14 I had this too and still do a bit (7.5mg now). It gets easier though. Once you eat less consistently your stomach shrinks and you can't put away huge meals anymore. On those days I go for smaller high protein, high fibre meals to feel fuller. A lot is willpower. Now that I'm 18kg down I treat hunger pangs like proof I'm winning.
