Un-reconstituted vials should be clear or slightly off-white - cloudiness or particulate before mixing warrants a supplier inquiry.
 
The vacuum seal integrity check is the primary indicator for untampered product - a properly sealed vial will have visible resistance when the cap is first broken. Powder appearance with no discoloration, clumping, or unusual particulates is the secondary check before reconstituting.
 
The flat cap versus ridged cap difference is a manufacturing variation rather than a quality indicator - different production runs and suppliers use different crimp designs, and the visual characteristics don't map to compound quality inside. The actual assessment is functional response to the vial you have used.
 
the floater question is the right one to ask first rather than just assuming safe - particles in a reconstituted solution can be protein aggregation from improper reconstitution technique, undissolved peptide, or actual particulate contamination. the three look different visually and behave differently when you tilt the vial. uniform fine cloudiness that clears is different from discrete particles that stay put
