Optimal Reta Dose?

I'm taking 1.5mg Reta once a week right now. Hunger is pretty manageable, but I notice it creeping back near the end of the week. I've seen others using much larger doses, like 6mg or even 8mg, and then the crazy high clinical trial amounts. Am I not taking enough? Should I slowly increase the dose each week if I feel it's becoming less effective? I was also thinking about splitting the dose and injecting half twice a week instead of the full dose at once. This is all pretty new to me so any guidance is appreciated!
 
Don't rush it! My wife started at 0.5mg because she was worried about side effects. If you've been on 1.5mg for a couple of weeks already, try bumping it up to 2mg for the next two weeks. Then, increase it by 0.5mg every two weeks until you feel like the hunger isn't getting to you. For example, I inject Sunday night, and if I start feeling hungry again on Thursday, I know it's time to increase the dose again. If the hunger doesn't come back until Saturday, I stay at the same dose.

Splitting the dose isn't really necessary to make it more effective, IMO. It's more about reducing side effects. I think taking 3mg in one shot each week is better than splitting that into two 1.5mg injections every 3.5 days (assuming you don't have noticeable side effects). Splitting won't hurt you, though. I just wanted to give you my perspective.

It's good that you're being patient. People who take their time tend to be more successful in the long run. Think about doing a gradual increase; there's no need to rush to the highest dose possible if a lower dose is already giving you results.

There are a few ways to figure out your dosing:

One way is to aim for a specific target dose to produce maximum weight loss. The other, is take the minimum dose possible to still see progress.

Another, more popular method, is letting your hunger guide you. While this may seem intuitive, you could end up chasing an impossible goal, similar to using pain medication until the pain is completely gone.
 
That's funny, I'm the complete opposite. I can only sleep for like 4-5 hours, but I feel totally rested. Then again, I don't have a newborn on my mind like you do, so that might be a factor.
 
Another thing people often miss is that our goals are different from those in the clinical trials. We want weight loss. Let that be your guide. If you're losing about 1-2 pounds a week, I wouldn't change anything. If you stall for a week or two, it might be time to titrate up, or it might be a good time to re-evaluate your maintenance calories since your TDEE decreases as you lose weight.

I also know it's fun to see the scale go down, and there's a dopamine rush from that. I want to increase my dose, but I know I don't really "need" to.
 
I'd recheck your vitals, especially heart rate. If side effects are a problem, think about switching to tirz.

The real question is, are you happy with the progress you're making right now?

If you're happy with the progress, there's no need to increase the dose.
 
Hey everyone! I started about two weeks ago. Most people say they increase to 5mg after like 4 weeks, but my pharmacist automatically filled 12 doses of 2.5mg. Should I stay at the lower dose until I've used all 12 or should I ask my doctor? Any advice?

Stick with what's working! If you're seeing results at 2.5mg, there's no need to rush to a higher dose. The goal is to use the minimum effective dose.
 
Yep, agree with both of you. No point cranking up the dose if what you're on is still working. I'll reassess in a few weeks. Thanks!
 
20mg Reta in active use confirms the ceiling is flexible - for women the range typically tops out earlier, so hunger creeping at week's end at 1.5mg usually points to a bump to 2mg.
 
The volume question depends entirely on the concentration - 40 units of a 2mg/mL prep is 0.8mg which is a reasonable starting dose, while 40 units of a 5mg/mL prep is 2mg which is higher. Always check what you're drawing against the concentration on the vial before assuming the unit count tells you the dose.
