The Indian generic market is the most significant near-term development for access and cost. Patent expiry timelines vary by country, but once generics hit the US market the pricing picture changes substantially. Compounding pharmacies have already shifted the landscape; the generic pharmaceutical wave is the next stage.
 
The deductible math is where most people get surprised - the starter pen hits at full cash price until the deductible clears, and then subsequent fills can drop to zero depending on how the plan is structured. The integrated system example is exactly how those plans tend to work: they negotiate their own formulary pricing and the member sees a completely different cost than what an outside plan charges for the same medication.
 
the T2D diagnosis is the coverage unlock that makes the cost situation completely different from weight management only - most insurance plans that don't cover GLP-1 for obesity will cover it for T2D with minimal or no cost share. the Walmart pharmacy contract pricing in some plans also produces better out-of-pocket than other pharmacies even when coverage is partial. worth checking which pharmacies are in-network for your specific plan
 
I use an online prescription service that ships straight to a pharmacy nearby, ended up paying just under 90 dollars for my 4mg pen. These services also run referral promos sometimes that knock the price down further.
 
My very first Ozempic script got filled recently, only ended up paying $124.99. Never figured out where the extra $50 discount came from, but wasn't about to complain about not getting charged for it!
 
Always route the prescription through a local pharmacy to maximize savings. Believe a particular generic telehealth option runs around 125. But going through a warehouse club pharmacy instead, ended up paying just 85 for the branded version.
 
Paying $110 for the 1mg generic through my pharmacy these days. Used to shell out around $240 for brand Ozempic back in Quebec. Not sure if that price-match thing applies here or not.
