Something I keep noticing with friends who look like they're wasting away, and it isn't negligence, they just were never told. Going on these meds without changing how you eat is risky. You eat less, so you get less of whatever added the weight, but also less protein, and protein is what holds your muscle in place. If you aren't deliberate about protein within a small calorie budget, you can lose a lot of muscle, sometimes enough to damage your health.
Lifting can't replace protein. Training breaks muscle down and the body rebuilds it stronger during recovery, using protein as the main fuel. In a protein deficit there's nothing to rebuild with, so lifting alone just makes it worse. Track your food in an app for a while because protein isn't intuitive until you build a sense of it. Honestly I think nobody should get a prescription without real nutrition counseling, not just a quick 'eat right'.
