40 lbs in 60 days off a 325 start is aggressive reta response - the nausea pattern through dose titration at that speed is expected. The real question at 20mg is whether you're hitting the dose ceiling or still in the response window.
 
14kg from 78.8 since February at the low starting dose is a strong response rate. The gap between 64.1 and a 49kg target is still meaningful but the trajectory is clearly working. The OP starting at 15mg is an extreme dose for comparison - most people get there eventually through titration rather than starting there.
 
The scale not moving but body composition shifting is one of the harder patterns to interpret early on. Muscle gain alongside fat loss can mask the actual progress. Comparing timelines and starting weights matters a lot when results look so different between people.
 
245mg total over 20 weeks tracking .2lb/mg lost is methodical data. The Tirz comparison at .8lb/mg shows a meaningful efficiency difference per unit for your response profile. Starting from a high entry dose is a different experiment than standard titration - the efficiency math is interesting.
 
Getting the actual scan data alongside the scale number is the complete picture. The body composition side often shows what the weight doesn't capture, especially when workouts are consistent at the same time. 40 lbs in 60 days in the OP is remarkable. How is your scan data tracking against what you expected?
 
Just adding to this. Picked up a 12mg kit, started running it back in October. Just reconstituted vial number nine last night, and that's even with my wife dipping into my last two.

Started at half a milligram every three days and worked my way up to 2mg every three days. Then adjusted the spacing instead of the amount. Currently at 2mg every six days. Starting weight 220, current weight 167.

The next couple of kits I've got coming are the bigger size, which had me worried the same way it worried you, but now that my wife's starting too, I'm feeling less uneasy about it.
 
This mirrors exactly how I got started with the second compound. Different provider, same basic setup. Found them through a social media ad. Ran through the standard onboarding, goals, health history, the usual telehealth intake stuff.

Got prescribed a month's worth (1mg weekly). A nurse practitioner attached to their platform handled the actual prescribing. It came from a compounding pharmacy and arrived already mixed and ready to inject. Packaging was solid, cold packs, syringes, alcohol swabs included.

The paperwork noted it wasn't FDA-approved but nothing on the vial itself mentioned research-only use. There may have been more detail on the prescription sheet but I didn't keep it. Opening month came at a reduced rate, then jumped to $399 monthly after.

Schedule ramped from 1mg weekly the first month, to 2mg in month two, then an increase of 2mg each month after.

Honestly this feels like a preview of what things will look like once it gets full approval. Onboarding was smooth and well handled. Only thing missing was a certificate of analysis, though given it's a licensed compounding pharmacy operating in the US, I'd expect they're held to real standards.
