Reta - push to 4mg or stick to 3?

Okay, so I'm in week 8 using Reta, and this is the 1st week where the scale hasn't changed! The initial month I was on 1mg, the following month I went to 2mg, and reduced 2 pounds every week until now.

I planned to move up to 3mg for the next month, but after not seeing movement this week, I'm thinking about bumping straight to 4mg?

My plan was a week at 3mg and an 8 week course of Tesamorelin started in week 2 at 3mg.

Do I follow through with the original plan, or just go straight to 4mg, which wasn't planned for another month?

Just sharing my experience... I went from 2.5mg to 4mg and I saw a noticeable difference in weight reduction. My titration was 1, then 2, 2.5, 4, 5, and currently at 6.

Personally, I would stay at 3mg for seven days, see where things are (did the scale move, any bad side effects) and then if still not happy, move to 4mg the following week.

You can increase the Reta dose relatively faster. You've been pretty cautious so far.

I'm curious about your tesa plan. What made you decide on 3 mg for 2 months? I'm not criticizing, but I've never seen that dosage for treatment. I've usually seen and used 2 mg, five days on/two days off (to let receptors reset, which is important) for half a year. This matches how Egrifta (tesa brand name) is prescribed. 3 mg won’t hurt, but from what I remember reading, there was no better visceral fat reduction at the 3 mg dosage during clinical trials. (I just saved you 33% on your tesa bill.)

When I titrated from 2mg to 4mg, I was losing around 3lbs weekly. Today's my 4th pin of 4mg, and next Monday I'll decide if I want to stay on 4, or go to 6 -8mg, depending on what the scale says. The stomach issues are basically gone after being on Reta for 2 months. I started a low carb diet on 1/9/26. I don't track macros or calories. I have an egg and sausage for breakfast, lunch is a shake, and dinner is some kind of meat, cottage cheese, sliced tomato or some steamed veggies. I usually have some beers daily. I started at 215lbs, and last weigh in was 195lbs. My goal is 175.

The answer is dependent on the negative effects you're having at your current dose. If you're having side effects go up slowly to 3mg, if no side effects go to 4mg. Neither guarantees you won't have increased side effects at higher doses, but at least that makes sense.

I agree with MacroGuyPlus that it probably isn't going to matter much... I would say that it is relatively normal even with GLP1's to have a week where there's no loss even with calorie reduction. If it's been a couple weeks, or you're weighing often and tracking the numbers, we could look into it more.

Also, it bugs me that there's no mention of diet/calorie consumption in these threads. It's a core part of this whole process.

If you've been losing 1 kg each week, the current dose is working. Not losing for one week isn’t a plateau, it’s normal fluctuation. Increasing doses too fast often makes side effects worse and decreases the med's effectiveness. Slow and steady is what works here.

Going up slowly is probably better, but it's not to say you can't bump to 4; the unwanted effects could be a bit stronger. Have you changed your diet recently? Adjusting diet sometimes changes things.

100% correct! It's only been 8 weeks and this is the first week without a loss. It's easy to react emotionally. Don't get shaken by it. Play the long game, not knee-jerk reactions. Always be cautious. Go slowly for better long-term results.

Thank you all for the replies. I appreciate all the advice.

It was kind of a quick post, and I was doing it while working. I should have added more information regardless.

Tesamorelin will be at 2mg 5/2. The original post was poorly written.

Starting weight 105kg
Current 98kg
Target 90kg

800 calorie deficit
Weights x 3 each week
Cardio x 2 each week

I'm away for the weekend, so my diet will be different, which might be a good reset for my body?

I think I'll go to 3mg as planned.

I saw someone microdosing to start to avoid side effects, like, injecting one little tiny unit to see what would happen and upping it slowly. I think they said they didn't have much weight to lose, but were hoping it woud help with the food noise.

I read somewhere about stretching the time between doses, like instead of every 7 days, some people do every 10 days or even longer. I wonder if that affects how well it works?

I think the general advice is that it's better to lower your dose and stick to the 7 day schedule if you can. Someone mentioned that 7 days is based on the half-life of the drug and keeping a steady level in your system.

Week 8 stall on Reta is common. If hunger control is still working at 3mg, hold. If it has slipped, bump to 4mg.

Trial titration schedules are built for population averages. Week 5 at 4mg is aggressive. Most people land somewhere between the study protocol and their own ceiling.
