My RS took 4 doses of 2.5, one dose every week, then after the fourth dose waited less than a week and took 2.5 again intending to take 2.5 twice a week to dial back the side effects. One poo after a few days and the next couple days later.

I keep reading about people who do 2.5 every five days to stay on the lower dose... so maybe try waiting a few days and see if it gets better?

RS wasn't even hungry today and didn’t eat till after the issues started, then just had a little. Maybe that's why?

RS says if this keeps up they can't stay on the meds since it has already cost them a day off work. I'm not sure what to do or what to recommend to lessen the side effects. Any advice for my RS?

Oh yeah, RS takes one mag o7 a night to avoid constipation (bad experience with Ozempic) Maybe that's the culprit? The magnesium totally fixed their headaches so stopping that would be a bummer.
 
Given the dosage, I think your RS is taking Tirzepatide?

Since Tirz affects people differently I would look at the food they ate over the past day or so, anything high-fat or greasy? I'm guessing it's the Mag7 (isn't it a cleanse?).

My RS had a bad experience after eating a serving of fatty smoked meat plus a glass of wine and magnesium. RS has lowered their mag intake since then and hasn't had the meat. The experience lasted a day and RS was on either 2.5 or 5mg Mounjaro at the time.

My RS had some butter chicken last night, a spicy sauce might have been a bad call but it is fairly lean.

The mag o7 is weird for my RS. Sometimes one doesn't do anything. Sometimes RS takes two and nothing happens then tries three and gets wrecked.
 
Did you increase your frequency because your current dose wasn't working, or just because you thought you needed to on a standard schedule after a month? If the 2.5 weekly was still doing the job and you weren't having problems, I'd just dial it back to that dose.
 
2.5 stopped being effective so I was trying to increase to the right dose after the first month. I’ll try 2.5 every five days from now on but I’m kind of afraid to keep going after how bad this has been. It’s kinda like norovirus without the vomiting.
 
That sounds terrible. When I first started going up in dosage I didn't have the same issues as you, but I couldn't eat anything and had constant sulfur burps for like 3 days. Then I decided to increase my dose by a tiny amount, and the next time I did 6 days for a couple weeks, then 5. Some of us need to increase the dose very slowly.
 
I'm so sorry to hear about your RS's experience, ToneBetter803. It's definitely not uncommon to have some GI upset when titrating up, but that sounds pretty severe. I wonder if splitting the dose even further might help? Like 1.25mg every 2-3 days? It's something to consider discussing with their doctor. Also, I totally agree with Fit_Path about going super slow. Some people just are more sensitive.
 
Oh honey, I feel your RS's pain! I had a similar experience when I first started on a GLP-1. The runs were SO bad. I found that keeping a food diary helped me identify my triggers. Dairy and fried foods were HUGE no-nos for me. Also, I second the suggestion to go super slow with the dose increases. It's a marathon, not a sprint!

This is something to watch out for, especially if your RS is experiencing abdominal pain in addition to the diarrhea. It might be worth discussing with a doctor, as gallstones can sometimes mimic other GI issues.
 
Frequent episodes of diarrhea can significantly impact electrolyte balance. It's important for your RS to stay well-hydrated with fluids containing electrolytes. Oral rehydration solutions can be helpful. If the diarrhea persists or becomes severe, medical evaluation is recommended to rule out other potential causes and prevent dehydration.
 
Wow, thanks everyone for the helpful replies! I really appreciate it. I'm going to pass all of this info along to my RS. Gonna look into electrolyte drinks, and slow the titration way down. I'll keep you all updated!

This is SUCH a good point. I've been reading about that more and more. It's definitely something to keep in mind on this journey.
 
I agree 100% with Dakota1976 about the gallstones. I actually ended up in the hospital a while back, and it turned out to be gallstones related to the weight loss. Not fun! If your RS starts having upper abdominal pain that comes and goes, definitely get it checked out.
