Splitting my dose?

Hi everyone!

Need some advice. I started on August 6th with a 2.5mg dose. I halved it to 1.25mg because I'm usually sensitive to meds. Didn't expect much, but I did notice a slight dip in appetite, enough to help me make better food choices. But today, the 9th, I'm starving! Probably because I've only eaten around 750 calories the last couple days (stomach's rumbling!). I plan to eat more like 1200-1500 a day, just wanted to start low to avoid feeling sick.

Would it be alright to take the other 1.25mg today, or should I wait a few more days? Since the side effects have been mild, I'm thinking of just taking the full 2.5mg next week.

Thanks!

Okay, so if you split the dose and take it every 3.5 days, the lowest level of the drug in your system will be about 61% of the highest level. That's assuming a 5-day half-life. Now, I'm ignoring how the drug gets released into the body after the shot because I couldn't find that info in the prescribing info. If you follow Eli Lilly's advice and take it once a week, the lowest point will be 38% of the peak. You'll hit the peak right before your next dose, assuming you take it at the same time each week.

I did notice that the manufacturer only seems to be interested in once-a-week dosing. All the trials used that method. I'm guessing if the drug worked way better with more frequent dosing, they would have recommended it. That being said, I'm mostly sticking with once a week because my insurance covers Zepbound. I'm still ordering compounded stuff to make the transitions smoother. For example, I'm currently using one 2.5mg pen plus 1.2mg of compounded while waiting to move up to 5mg. Is easing up with intermediate doses better than just jumping to 5mg? I have no idea, but that's what I'm doing.

I started splitting my 10mg weekly dose into 5mg every 3.5 days back in May, after already doing 10mg weekly for 2 months. I noticed it worked even better, with stronger appetite control, less food noise, and slower digestion that lasted consistently for the whole week. I'm still splitting the 10mg and just this week going up to 12.5mg, and I'll split that too.

I split my 5mg into two 2.5mg doses every 3.5 days for the same reason. And I use Shotsy to track it.

You'll experience the peak immediately before you administer the next dose

Did you mean the peak or the bottom of the trough on the standard 1x/week schedule? You are almost at the bottom right before the next dose. It takes hours to reach the peak after taking it.

I wouldn't assume the recommendation is because it works better that way. Two big things for a new drug are ease of use and compliance. Suggesting a 3.5-day schedule might confuse some. Missing a dose is more problematic with only 3.5 days between them. People miss doses all the time, ugh.

Also, the pens are pricey. Needing more pens would lengthen the shortage, costing them more money. Same with new single-dose vials. More frequent dosing would mean more research expenses to compare once vs twice per week, and why invest if they don't want to market it that way?

Maybe I'm cynical, but I think money speaks loudest – louder than maximum efficacy. Some say it's recommended at 1x/week to let the body cycle down to the lower trough, but I can't confirm if that's true or just a theory.

I agree with @KelsKetoKlub about the marketing and compliance aspect. Simpler dosing schedules are always favored, even if they aren't optimal for everyone.

I lost close to 85 pounds in 10 months! It was a journey, but so worth it. I cut down on carbs, and focused on upping my protein intake with every meal. Gym 4-5 days a week, cardio plus some weights.

What dose are you at right now? Some folks think 3 months storage on reconstituted tirz is pushing it. If you've got a partner splitting, you're probably fine. I've seen 2 months max as a safe window in the fridge. Light and temperature swings break down the peptides - every in-and-out exposure matters.

I split mine as well. Notice a drop by day four rather than day six like the studies mention. Splitting keeps my levels steadier - fewer peaks and valleys. Really helps cut down side effects. If once a week is working for you, awesome. But if you feel the decline coming around day four, there are other choices.

Splitting 12 to 6mg twice weekly is working. Could go daily or every other day no problem, just more pinning. Cagri and teso arriving soon, starting super low with those.

Interested in this. Doing 3.5 of one after three months 5 of another and get stomach acid 24 hours after every 3.5 dose. Going to 4 this week thinking about splitting 2 Thursday and 2 Sunday to see if the acid stops.

been on 2.5 for about a month and dropped 9 kilos, no sides at all. feeling great. doc wants me to move to 5 but honestly i'm crushing it here. wondering if i should just stay put or if half-dosing the 5mg pen is even a thing? wish i'd asked her to let me hold steady.

wanted to hit 6–8mg steady state without flooring myself with side effects. smaller doses could accumulate with a small peak, then clear quick. started low, evaluated sides, kept going till I hit my target, then switched to 2x weekly.
