Splitting Tirz Dose: Good or Bad?

I was taking 5mg weekly, but had really awful diarrhea for a couple of days, with a day to recover. So, I've decided to lower my total weekly dose and split it into 1.5mg twice a week, gradually increasing it. I saw something online saying this might help with side effects if monitored by a doc. Am I missing something? Splitting the dose seems fine to me – lower peak, higher trough. Is this just doctors trying to keep control, or are there actual risks I'm not seeing?

I had to split my 7.5mg dose too! Around day 4, I'd get sulphur burps, the runs, and incredibly painful stomach cramps that made me miss work. It only felt okay when I was sleeping, thank goodness for the exhaustion! The worst 24 hours ever, then BAM, gone. You'd think I'd learn after the first time, but nope, it took 2 more tries before splitting the dose lol. Pepto was the only thing that helped me work almost a full day during the last episode.

It definitely smooths out the peaks and valleys. I'm not sure if there's research on whether the weekly spike is beneficial or if steadier levels are better... You might want to adjust the bi-weekly dose to match the average level of the weekly dose instead of the peak. This dosing tool might help (thanks @Cut for showing me this!):

I used to inject Tirz every 5 days because I got too hungry on days 6 and 7 and messed up my weight loss. Now I'm on reta twice a week to avoid the peaks and reduce side effects. It works great, zero sides. But yeah, not tested in trials, so who knows. Lots of people split doses though.

There's a calculator on Fatscientist that tells you exactly how much to inject for your desired dose. If you want the equivalent of 5 mg/week, enter that and how often you want to inject, and it'll tell you the mg amount. I inject every 5 days, and it helps me figure out how much I need so I don't go over my weekly amount.

This is neat. I was on 15mg weekly, then went to 12mg every 5 days, and then 7.5mg every 3 days. It was all guesswork using shotsy to see my average levels. Turns out I was close – I'm on an average of 17.5mg weekly!

I'm splitting my dose to try and curb hunger on days 5, 6, and 7. I inject 4mg on Monday nights and 4mg on Friday evenings. It's working pretty well. I still think about food, but not nearly as much as when I injected once a week.

I inject daily, and I'm not sure how a doctor would monitor more frequent dosing. If you use the brand name pens, splitting is physically difficult (I've done it with Zepbound, but it's not easy). Even though I inject daily, one argument against more frequent injections is that Tirz was tested weekly, not more often. My side effects are less, and I have a more consistent lack of appetite on a lower dose because I avoid the valleys of weekly dosing.

I wonder if splitting doses is why I haven't lost as fast as some other peeps. I've only dropped maybe 10 pounds in a few months. But honestly, I'd rather lose slower and not be stuck on the toilet all the time! Maybe I should try going back to the single dose and see what happens?

I'd stick with tirzepatide as long as it keeps working. Really solid option overall, but the high-dose depression hit me hard, so that became a dealbreaker. If side effects are rough, dropping the dose might help, or testing out 6 mg could be worth a shot.

That sudden appetite surge sounds incredibly frightening and definitely occurs regularly when folks taper or discontinue these medications. Being hit with that on the highest tirzepatide dose would be awful. My sister skipped one weekly shot a couple weeks prior due to nausea and I heard nothing except complaints about hunger the entire week.

man, 4 days of irritation is rough. mine was maybe a couple hours or i go straight to bed so who knows. all gone now though. antihistamine before the shot might help you. saw someone have a real bad allergic reaction to another med and hit the hospital. if it gets worse, maybe switch it up.

you're not at therapeutic dose yet, give it time. yeah, you'll still crave sweets — they're easy to digest so your brain tricks you. I'm over here snacking chocolate raisins after a salad for lunch. all the best on your journey!
