Switching from Tirz to Sema - experiences?

Hi everyone, I'm making the switch from tirz to sema because I found the tirz a bit too intense. Dropped around 22 pounds in 2 months using diet and exercise (yay!), but the nausea, stomach issues, and funky breath were a real problem.

I've got 15mg of sema on hand. Two questions:

1. How long have you all kept reconstituted sema in the fridge? I'm figuring around a month max, but I think I might have ordered more than I need even then.
2. Anyone notice a drop in effectiveness when switching from tirz to sema, especially with time in the fridge?

If you were on 10mg of tirz after only a couple of months, it's no surprise you had issues. Ramping up that quickly is usually not a great idea. If you try that with sema, you'll probably feel even worse. You should try lowering the dose.

Like @Tired_of_Tired said, sounds like you increased too fast. Maybe you don't need to swap to sema, just cut back on the tirz. And I'll second that – going too hard too fast with sema could be much worse.

What dosing schedule were you following on the tirz? If you were going by the typical one, you'd only be starting 7.5mg after 2 months. Listen to your body!

Great points here! I agree that semaglutide can be rough for some, potentially more so than tirz. If you decide to stick with sema, absolutely follow the recommended dose increases. Semaglutide can be very potent; gradually increasing your dose lets your body adjust and minimizes side effects.

I was on it longer than two months, but once I hit 10mg, it was too much. The weight loss was great at first, but the side effects started to outweigh the positives.

Switching medications can be tricky. It's important to remember that these drugs, while effective for weight loss and glycemic control, have potential side effects. The half-life of tirzepatide is around 5 days, so any adverse reactions could linger for a while. Semaglutide also has a significant half-life. Make sure to titrate slowly and monitor for any concerning symptoms. Has anyone experienced gastroparesis with these meds? I'm curious about long-term GI effects.

Wow, some of these doses sound HUGE! I'm just starting out. Is it normal to feel like you don't want to eat at all? I'm only at 5mg of tirz and feeling kinda full all the time. I hope I can still lift weights...

@JerseyGirl_Health, yeah, feeling full is pretty common. Some people lose their appetite a lot. You might need to adjust your diet to make sure you're getting enough protein and calories, especially if you're lifting. Some people feel weaker on these meds, so listen to your body.

I think the key, as others have mentioned, is slow titration. I had a rough time when I increased my sema dose too quickly. Nausea, fatigue... it wasn't fun. Now I increase very gradually, and it's much more manageable.

Thanks for the info! I'll definitely be keeping a close eye on side effects. I'm hoping the sema will be a bit gentler on my system.

Also, to @JerseyGirl_Health, feeling full is a common side effect. Listen to your body and don't force yourself to eat. Focus on nutrient-dense foods. If you experience persistent nausea or vomiting, contact your provider. There are medications that can help manage those side effects.

never heard that one. could be a couple things though - absence of sensation vs actual numbness are different. i have sciatica and the tightness is gone since on tirz and losing weight, probably less swelling. cold also feels like numbness sometimes. both sema and tirz make people's internal thermostat go cold. could be either one.

Lost 40 pounds over 16 months - went from 193 to 153. On 5 feet tall that was way too heavy for me. Year on semaglutide then four months on tirzepatide. Target is 145, stretching for 130 but progress is slow.
