Your doctor is right, a well-established titration protocol exists, and pens are prescribed around it. My doctor and I do something off-label but not rare. Cost worried me, so I suggested a bigger pen and counting clicks for smaller doses. After I understood the process and the math and took on the off-label risks, my doctor agreed and wrote a script for a 15 mg pen with a note to give 5 mg doses. The pharmacist confirmed I understood click counting, and I got the pen. Seven weeks in, I use it for 5 mg doses, which costs way less. He is an endocrinologist, referred by my liver doctor.
