Tirz plateau - what should RS do next?

I've been reading the forums for a little bit and you all have such great insight! I wanted to finally jump in and ask for some advice.

My research subject (RS) had a VSG in February of this year and is down about 65 lbs since then, plus another 20 lbs since starting on tirzepatide. They're trying to drop another 40 lbs or so to reach their ultimate goal.

RS has been taking 5 mg of tirzepatide weekly for the last month or so, after an initial month at 2.5 mg. Now they are starting to get some cravings and food noise creeping back in, especially in the few days before the next shot. RS is thinking about these options:

1. Going up to 7.5 mg next time.
2. Splitting the dose into 2.5 mg twice a week.
3. Taking 5 mg every 5 days instead of 7.
4. Adding in a small dose of cagrilintide.
5. Switching to retatrutide until they hit their goal, then going back to tirzepatide for maintenance.
6. ? Any other ideas are welcome!

I never went higher than 5mg on tirz. I started stacking with retatrutide when the cravings resurfaced around day 5. I then switched to retatrutide entirely and it's been fantastic. I have unused tirz in the freezer for maintenance if I ever need it.

If you haven't had any adverse effects with tirz, you could try a higher dose and see if that solves the problem.

Or, splitting the doses might keep the food noise at bay.
 
I think all your ideas are worth trying, but I'd be careful with cagrilintide. I'm seeing some concerning stuff about it on other sites. Even though it performed well in studies, the company seems to be abandoning it for something different.
 
I hadn’t heard they were discontinuing cagrilintide! Where did you find that info? I'm not doubting you, I'm just curious.
 
I'm also curious about the source. I'd stop taking it immediately if that's true. I only spent a little on my current supply, so it wouldn't be a huge loss.
 
I'm currently doing option 3 and it's working well. Spacing out the doses to every 5 days seems to minimize the food noise. I'm also interested in any updates on cagrilintide, since I planned to use it for a reset down the road.
 
I'd try 2.5 mg twice a week until your weight loss stalls for a month. Then, try 5 mg every 5 days. I've had good results injecting more frequently at lower doses (every 3-4 days instead of once a week).

No problem at all. Just dial to 1mg, but don't push the button. Count the clicks as you dial it back down. Then when you dial back up, you'll know exactly how much you're getting. You'll need extra needles eventually, but it works great.
 
Met with a dietitian my prescribing endo set up and it was rough. Told her I was losing muscle according to my smart scale. She just handed me Abbott samples with HMB in them and said take HMB. Never talked about calories, never talked about protein, never talked about fiber. She's got me booked for early May to do indirect calorimetry to test my metabolism. Worth going or should I skip it?
 
If you bumped from 2.5 to 4 you should start noticing it pretty soon. Stick with it a bit longer and try stepping up to 5 on your next shot if nothing's changed.
 
Day 7 on 0.5mg MT2 now, moving to twice weekly for upkeep. Appetite suppression hasn't kicked in yet. Planning labs next week - wondering how often I should check in after this? The last few days showed a slight gain which could be why I'm stuck.
