Tirz split dose + sema... how to?

6mg every 5 days and 5 lbs left that will not shift is a really common landing spot. Adding sema at a low dose alongside the tirz is a reasonable next move before bumping the tirz higher. What dose were you planning to start sema at?
 
The receptor blowout concern hasn't held up in practice at any dosing level that's been reported in the community or the clinical literature. The systemic mechanism these medications use doesn't create the tolerance pattern people worry about. Stacking for added GLP receptor coverage is a documented approach and the split-dose method is the right way to manage the pharmacokinetics when you're on a higher weekly total.
 
The once-weekly versus split dosing question comes down to whether the trough at day 6-7 is causing enough hunger to justify the more complex protocol. Some people find split dosing smooths out the end-of-week hunger spike significantly; others find the single weekly pin gives consistent enough coverage. Worth trying both to see which pattern works better.
 
The twice-weekly split improving appetite control is consistent with the pharmacokinetics - coverage is more even across the week with two smaller doses than one larger one, which avoids the end-of-cycle dip. Sunday and Wednesday spacing is close to optimal for distributing the half-life.
 
Personally I'd push Reta higher first before layering in anything new. Got up to 20mg of Reta before adding .25mg of Sema.

Hit my first target weight this week, currently tapering the Reta amount back, missed one of my split doses and I'll take 6-7mg Monday instead (splitting doses is my usual approach). Haven't bumped Sema in a week, waiting to see if symptoms return, mainly that food chatter.

Went with Sema since I hadn't tried it yet, and it's given me the roughest side effects by far, though relatively speaking still fairly mild, mostly showing up whenever I tried increasing the amount... Originally planned starting at .5mg weekly but that turned out to be a mistake. That very first .25mg dose completely silenced my food noise. Ended up splitting .125mg twice weekly, occasionally bumping to .1875mg.

Basically, until side effects become the limiting factor, my general approach is to keep escalating to hold onto effectiveness.

I'd push Reta further, at minimum reaching 12mg weekly. Then add in a single milligram of Tirz on top, watch how the body responds, and adjust as needed.
 
Began layering Sema into a solid Reta routine, hits the eight-week mark tomorrow. Total intake across that stretch comes to 1.8mg Sema (roughly .225mg weekly average) and 118mg Reta.

Down a clean 20lb, though part of that's likely just water loss from quitting creatine. Side effects have genuinely ramped up since adding Sema into the mix... nothing objectively terrible overall, just the roughest stretch I've had since first starting on Tirz and later moving to Reta.

Ramping the dose matters a lot. My very first shot was .25mg and it wiped my appetite completely while leaving me feeling generally lousy, pushed back the following Reta dose until things settled. Been splitting .125mg twice weekly ever since that first shot. Sunday I moved up to .1875mg and Monday marked my first time throwing up in years. Was swallowing capsules at the time, might've irritated my throat somehow, so can't fully pin it on Sema... but I did feel that same lousy sensation stronger, so Sema's probably the real culprit.

Anyway, I've got well over a decade's worth of Sema stocked away. Considering shifting over to Tirz entirely, hear it's a much gentler ride.
