Tirz... Stuck. Help me decide!

I'm struggling with my next step on 2.5mg tirzepatide...

I'm waffling between:

A) Sticking with 2.5mg every week to see if the effects increase over time.

B) Shortening the interval to every 5 days, still at 2.5mg.

C) Bumping up to 3.5mg weekly.

D) Making the usual jump to 5mg weekly.

I'm about to inject my 4th dose tonight. It's curbing cravings, but I'm still getting hunger pangs, mostly around days 5, 6, and 7. I'm generally pleased, but as I cut calories, I want to feel full for longer. I'm worried about the hassle of a 5-6 day schedule and keeping track, especially since my husband is starting this week.

Strangely, week two felt easier than this week, so I'm not sure another week at 2.5mg will make much difference... Thoughts?

I've been dividing my doses and injecting every other day, about 0.6mg each time. That adds up to around 2.2mg per week. I feel pretty good, a bit more energetic on injection days, and slightly less the day after. No negative side effects so far. I'm losing about 1 pound each week. I'm thinking of adding a tiny bit of Reta to see if it helps with fat loss.

I'd advise going to 5mg. The clinical trials support it. We know the success rates and the potential side effects. I'd stick to the established protocol until it stops working before experimenting. Sure, some people are splitting 2.5mg into multiple doses per week, or doing 0.75mg daily, etc. Those might be okay, and seem effective for some. But are they experiencing more adverse effects? And are they seeing better results because of superior protocols, or are they just lucky outliers? I don't know the answers to those questions.

When I began, I used the usual 2.5mg, then moved to 5mg, then 7.5mg, and then back down to 5mg. The 7.5mg gave me several side effects – couldn't eat, nausea, no sleep, and always tired. So, I went back to 5mg.

Tirzepatide's half-life is about 5 days. Try your current dose on that schedule. If it doesn't become more effective, titrating upwards would be my next move.

It's a dose-dependent response. Within the standard dosing range, a higher dose usually means more weight loss. You need to stay on a dose for about a month to reach a stable blood level. Clinical studies used once-weekly injections and achieved success. The target range, in my opinion, should be 10-15mg per week eventually if you need significant weight loss.

I'd suggest titrating up to 5 mg every 7 days. I was on 2.5 mg for a month and then went to 5 mg because I had more food cravings and very slow weight loss. I stayed on 5 mg for a month and then went to 7.5 mg. I stayed on the 7.5 mg for around three months because it worked for me. After that, I started 10 mg because of a stall. I have been on 10 mg for four months and my weight loss has slowed but I am still losing some weight and the food cravings have not returned so I am not increasing my dose right now. I have lost about 60 pounds and am pretty close to my goal so I am excited to learn maintenance. I use tirzepatide and was using a compound under my doctor's care for most of the time. The cost became too much so I had to research grey market options. So far, so good.

I'm with @Blair on this one. Sticking to established protocols backed by clinical trials is generally the safest bet, especially when you're just starting out. You can always explore the less conventional methods later if the standard approach doesn't give you the results you're after.

Thanks for the advice, everyone! I really appreciate it. I'm leaning towards sticking with the tried-and-true method and bumping up to 5mg. It's reassuring to hear that others have had success with that approach.

I always remind my patients getting on GLP-1s to hydrate much more than usual. The gut slowing down plus less food intake can really impact bowel movements. Stool softeners are helpful, and I've also had some patients find relief with magnesium supplements.

Has anyone had really bad constipation on tirzepatide? I've heard it's a common side effect. Any tips?

Something as simple as a daily fiber supplement can make a huge difference. Also, make sure you are drinking enough water. Dehydration makes constipation worse.

Just hit my goal, gonna see how it goes. If I can stay at 1 mg twice a week without food noise, that's the plan. Original idea was dropping to 0.75 mg twice a week. Right now I'm in a good spot and don't want to mess with it. Got TRT going so the goal is protein and lifting to gain muscle.

getting my mots-c in a couple days (cleared customs in bc). was gonna do 2.5-5mg 3x week, that's what everyone seems to do. but just watched a dr from a peptide conference say that this one should be microdosed daily instead (1-2.5mg). think i'll start 1mg tomorrow then daily. suggestions?

Lost 50 lbs on Wegovy at the highest dose, then gained it back. Started again and felt nothing until 1.7mg. Stayed there, moved to 2.4 about 2 months ago. If I plateau my doc'll switch me to Zepbound. Still dropping weight now, 40 lbs down with 50 to go.
