Tirzepatide and wicked acid reflux?

Hey everyone - I've been getting some pretty bad heartburn at night since starting on Tirz. Has anyone else experienced this? Does it get better, or is there anything I can do about it?

Woke up last night feeling like my throat was burning. Not fun.

I only took a 1mg dose about 2 days ago.

Nighttime reflux on tirz is common early on — a thread on GLP-1 GI side effects including reflux has what people did about it alongside the other GI stuff.
 
My husband takes a daily proton-pump inhibitor (PPI) that is available without a prescription now, and he says it's a total gamechanger for reflux. I've been on Tirz for over a year and haven't had reflux. He only started it a couple weeks ago, and it's been the only side effect he's had so far. I bet the PPI is why I've had no issues because I take it daily for my stomach.
 
I take a PPI and a probiotic starting the day before my shot and continue for three days after. I also take gas relief pills during those days. Also, I started using Buoy Digestion Drops.
 
I've had acid reflux for years. Tirz has made it worse, but I've started to figure some stuff out.

First - What you consume is important. Stay away from super spicy foods, foods with lots of garlic or onion, and acidic stuff. Booze makes GERD so much worse. Avoid foods hard to break down. Tirz slows digestion, so foods that are already difficult to digest make the process even slower. Use search to look up what to avoid while on Tirz.

Second - When you eat is important. Stop eating 3-4 hours before bed. No late-night snacks! Cut back on water, too. Tirz slows down digestion, so food sits in your stomach for hours, generating more acid. An empty stomach at night is ideal.

Third - How you sleep matters. Prop yourself up with pillows. Gravity is your friend. If you have an adjustable bed, raise both the head and feet a bit. Don't sleep on your right side or back; sleep on your left side in the left lateral decubitus position.

I've tried most of this already, but I haven't tried omeprazole yet.

I only took 1mg for the first week, and I didn't feel anything for the first day or so. Now I haven't been hungry for 3 days. I've eaten a little, though.

On night 2, I had terrible acid reflux, worse than ever, but it only lasted one night. I think it might get worse as I increase my dose, though.

Otherwise, it's doing what I thought/hoped. Also, bowel movements are a little weird. I think I've lost a little weight, but I also feel a little constipated.
 
Also, because gastric emptying is slowed, food sits in your stomach for much longer. Some people who've vomited are seeing food from days ago. Don't each spicy foods before bed!

You can find Omeprazole pretty easily, and there are also some teas designed to help with colon cleansing.
 
Famotidine works for me. It's worse on tirz, and it encourages me to eat less. Large meals just sit there and cause issues.

Nighttime GERD can also be a symptom of sleep apnea. My CPAP mask fixes it.
 
That's wild because my reaction is the opposite - being on tirz has almost completely cleared up my GERD/acid reflux symptoms. I used to constantly take omeprazole and another med, had a bunch of dr visits and tests and there was never anything found, just prescription management.

I started tirz last year and I might have a reflux “episode” once a week or less often. No more burping constantly, no random stomach pains, no more feeling like I was having heart issues when I lay down, no more palpitations and sweating. It's just gone.

I'm guessing my stomach has always been overactive, maybe a symptom of the need to always eat and think about food, and tirz is helping that.
 
Same here. I’ve been taking omeprazole for over a decade to help treat an autoimmune issue, and my acid reflux noticeably worsened when I increased my dose of tirz.

I did some research and figured out that the omeprazole might be making things worse when taken with a GLP-1. My autoimmune symptoms have disappeared as well! I'm planning to get some tests run soon.
 
I see some people suggesting colon cleanse teas. Be careful with stimulant laxatives; they can make your gut become lazy if you use them too much. It might be better to try something gentler.
 
I've also heard that sugar-free candies with sorbitol can help with constipation, but warning, it may cause GI upset if you eat too many! Always read the labels.
 
i eat like crazy 2 weeks a month but still way less than before mounjaro. worst is the week before my period. when i'm actually on it? can't face food. just accepted it as hormonal, it's way down from before so i'm good with it.
 
I tell most people I'm on Mounjaro when they ask. Nobody in my life would be rude about it anyway. With close friends I'll say it's compounded. Good friends get the full story - the pros, the grey market stuff, all of it. People need the truth, not silence.
 
The acid reflux follows gastric motility slowing - delayed emptying means food and acid sit longer in the stomach, especially lying down. Eating earlier and elevating the head are the two adjustments most people use.
