Weight Back After GLP-1s -- Inevitable?

Recommendation: don't stop without help. Statistics vary but most people regain weight, sometimes all of it, possibly more. I quit after one year and regained everything in a week, then depression made it worse. Rough stuff.
 
This realistically plays out as a long-haul, probably permanent treatment, unless there wasn't much excess weight to begin with. Solid long-term data specifically on this newer compound is still thin, but the other two established options in this drug class both show remarkably positive long-term effects, and those benefits will almost certainly extend to this one too, so staying on it isn't harmful, it cuts the risk of type 2 diabetes, elevated blood pressure, poor cholesterol numbers, heart disease, strokes, certain cancers, maybe even cognitive decline, and generally the heavier, older, and less healthy someone starts out, the bigger these effects tend to be. There's even some evidence from cellular aging marker research suggesting it might slow down how fast the body actually ages.

So for anyone overweight with health issues or obesity, staying on it is genuinely beneficial, and for those at elevated risk, higher amounts tend to outperform smaller ones. Beyond all that, ideally with this compound or any option in this drug class, the goal is really only losing the weight once, then staying on it just to hold that line, it's nearly that straightforward. Most people who stop after losing weight end up regaining it. Some prefer lower maintenance amounts, which works fine if side effects are a concern.
 
The real questions worth asking are how significant is the excess weight, and has it been lost and regained before?

If someone's dealing with substantial obesity, especially alongside health complications like diabetes, prediabetes, elevated blood pressure, high cholesterol, heart issues, even joint problems, there's a strong case for simply staying on treatment indefinitely, given the long-term health payoff both from the medication itself and from the weight loss it produces.

Unfortunate truth is most people who drop weight through any method end up regaining it once they stop whatever got them there, GLP medications included. That said, GLP drugs remain the single most effective tool for sustained long-term weight maintenance that's ever existed, short of bariatric surgery.

Sticking to a restricted-calorie diet to hold onto weight loss is genuinely tough, since losing weight makes you hungrier and also lowers how many calories your body burns at rest, meaning you need to eat below the average just to maintain, which is a hard ask. GLP medications ease that burden considerably, particularly over extended periods.

Having shed 'a substantial amount solo before' and still needing more off strongly suggests you put a good chunk of that back on at some point.

Dropped most of my weight going from 145 down to 75kg without any GLP help, held that for a full year, transitioned onto GLPs after that and eventually settled around 64kg by the second year in. Trying to hold weight off with these medications versus without them is night and day different, even though it takes a combination of higher amounts, roughly tirz at 15mg, reta at 6mg, cagri at 0.5mg. I've probably reached a normal or near-normal BMI something like six separate times across my life, but never sustained it this long before now.

Based on reading the fairly grim long-term outcomes from five decades of diet-and-exercise research, my take is that those approaches work fine short-term but the durable results basically never materialize. So personally I don't buy that intentional calorie restriction through sheer willpower or mental discipline holds up over time. It nearly always caves eventually.

On the flip side, unless you fall into that unlucky 5-10% who just don't respond well to these drugs, holding onto your progress indefinitely turns into a pretty solved puzzle provided you keep taking them. Personally, this
