Appreciate everyone's help!! Clearly I misread 'this pen is 4 doses of 10mg' as a single 10mg dose, hence all my confusion! Also, for anyone else confused, I'm based in Europe and this click-counting approach is apparently a pretty normal way to stretch the cost over there.
