Feeling sick all the time on sema?

I was on 0.25 of semaglutide for a month. Then I went up to 0.5 for 3 weeks but then I got a terrible stomach bug so now I'm back at 0.25. I've lost 17 lbs since Dec 20th but I always feel like I have some heartburn or like I'm gonna puke. Is this just how it's gonna be now? I'm glad the food cravings are gone, even at the lowest dose. Just wondering how everyone else feels. I think I'll go back up to 0.5 now that I'm not sick anymore.

Thanks for posting! Please remember our rules. We don't allow discussion of non-FDA approved versions of semaglutide, nor are we permitting selling any meds. Do not respond to any PMs offering such. If you're new, check out the FAQ.

I had to split my starting dose for a bit to avoid feeling awful. I never even went above the starting dose 'cause it worked so well. If you're still losing weight, maybe don't increase it? I've been on it for almost 2 years and now I take the starter dose every other week. Drink lots of water, eat fruit, vegetables, and protein (and less processed food). Peppermints help my stomach too.

My side effects on semaglutide weren't too bad, and I was seeing results. But I switched doctors and decided to try tirzepatide instead. It's been great, zero side effects! No nausea or anything! Maybe you should try that?

It took me a long time to get to the highest dose 'cause it was making me so sick. I'm not as sick as I was when I first started, but I'm pretty much always nauseous. Zofran helps a lot - ask your doctor for a prescription.

I stayed at the starter dose for 2 months and then went to 0.375 mg for a month and then to 0.4 mg, and I've been there for 3 months. I think it might be a little too high. I might go back to 0.375 mg. I've dropped 45 lbs in 5 1/2 months.

What are your eating habits like? That's what gets me... I've had stomach aches like three times but I actually threw up for the first time today.

During my first week on 1mg, I caught a bad cold and missed the gym.

Yesterday was my second 1mg dose. I had dim sum with friends and they commented on how little I ate (usually I could put away tons of food). I probably ate like 800-1000 calories.

I lifted weights for the first time in 9 days and felt fine until the end... then I did the last set and puked outside. I felt bad for like 20 mins... I'm gonna stick to salads and protein bars.

I agree with @DosePal. If the nausea is persistent even at lower doses, it's worth getting checked out by a doctor. Could be something else going on.

I know how you feel @DosePal. It can be a real struggle. For me, it's manageable with medication, but it's still always there in the background. Remember that everyone reacts differently, and it might take some time to find what works best for you.

So true! I find if I eat anything too greasy or sugary, I feel terrible for hours afterwards. Sticking to a healthy diet makes a HUGE difference for me.

Everyone's different. Been on semaglutide two years, down about 110 pounds. Appetite suppression was heaviest at first dose, lightened at higher ones, then back and forth. Never lost hunger completely, no side effects.

Regional illness bugs everywhere. Been sick way more this year than prior three combined. Skip doses when unwell. Manage hydration carefully since thirst is suppressed.

Stomach bugs on top of sema are rough because delayed gastric emptying means everything hits harder and lasts longer. Protocol when actually sick: pause or delay the dose if nausea and vomiting are already bad, focus on hydration above everything else, and hold at the current dose rather than pushing up once you recover. The GI sensitivity makes any illness feel more intense than it otherwise would. Worth noting if sick episodes correlate with dose increases - that's the diagnostic question.

Get another hobby. My mind has capacity for other things now. Picking up new activities helps loads. Years ago when depressed I'd shop a lot. Advisor said go to charity shops instead. Helped more than it helped charity. Still do that when stressed.

Arms have been reliable for me. Tried the thigh once and ended up nauseous and vomiting, so that's off the rotation. Never tested the stomach - no reason to at this point.

The 0.25 to 0.5 step is where sema GI tolerance gets tested hardest - 8 weeks at 0.25 before stepping up usually flattens the severity curve.
