The Metformin GI experience is highly individual - some people tolerate it fine and others find the diarrhea effect persistent regardless of whether they're on immediate or extended release. metformin plus tirzepatide or semaglutide is a common combination for T2D management and most people find the combination adds something over either alone. the mechanisms are different enough that they don't just overlap - metformin works on insulin sensitivity and hepatic glucose production while the GLP-1 handles the other side of the equation
