The Metformin GI experience is highly individual - some people tolerate it fine and others find the diarrhea effect persistent regardless of whether they're on immediate or extended release. metformin plus tirzepatide or semaglutide is a common combination for T2D management and most people find the combination adds something over either alone. the mechanisms are different enough that they don't just overlap - metformin works on insulin sensitivity and hepatic glucose production while the GLP-1 handles the other side of the equation
 
Your digestive system likely took damage a while back once diabetes set in. Diabetes doesn't reverse, just gets managed. What else are you taking for it currently? A common older medication's been around for decades and still gets used widely.

What about easing the dose down gradually, or spacing shots out every other week, and seeing how that goes?

Also an option to just stop entirely and observe what happens. Reportedly clears the system within a few weeks based on what's out there.

Been managing type 2 for close to two decades myself, spent the earlier years on an older medication alone, always landed in diabetic range on labs regardless. Switched over to this drug class a while back and later moved to a different one in the same family. Lost a substantial amount and labs now read solidly non-diabetic. Still on that older medication too, just at a reduced amount now.

Diet's changed some, but honestly think the shift in how food even registers mentally matters more than anything else. Sure, the weight loss played a real role in better blood sugar and dropping blood pressure medication too, but doubt everything holds steady if this medication ever stopped.

Plenty of providers believe there's added value, heart health among other things, to staying on this drug class long term. Plenty of skeptics too, of course.
 
Kicked off metformin toward the end of autumn, and my A1C's improved substantially, though I've packed on weight over the past half-year eating identically to before. Also deal with inflammatory allergic asthma and regularly spend weeks on steroid courses each summer, another weight contributor. Recent research suggests ozempic can help with that particular asthma type. Gave it a shot this past week accordingly. Coverage handles most of the cost, nowhere near what asthma biologics run.
