Sticking with the .25 dose?

Hong Kong Yanpep International
The 0.25 starting dose feels too low but the side effects window is real. Most of the worst hits in the first few weeks - nausea, fatigue, that vaguely off feeling in the 24-48h post-injection. A lot of people want to quit exactly when their body is adjusting. The week 1 drop is mostly water weight shifting as appetite drops and glycogen depletes. Weeks 3-4 show a slower pace that's more representative of actual loss. If side effects are the main issue, holding at 0.25 longer is worth it - dose bumps typically intensify things before they settle. Get to week 6 before drawing conclusions.

The 0.25 dose is designed for 4 weeks minimum before the question of titrating comes up - the first week 8 lb drop then 2 lbs is the typical settling pattern.

Staying at 0.25mg when it's still producing results is the right clinical call regardless of what the standard titration says - the protocol exists as a floor, not a ceiling, and letting your body's response guide the timing rather than the calendar is how you avoid the GI effects that come with forcing dose increases prematurely. The fact that 8 lbs dropped in the first week and continued steadily is exactly the kind of response that doesn't need adjustment.

The 8-week intro timeline is there because the first 4-8 weeks of loss is often water plus the early suppression effect - it can look dramatic and then flatten before the real trend shows up. Whether you're a super responder is clearer after the titration rather than in the first few weeks. 8 lbs then 2 lbs sounds like a normal progression, not a stall.

The 1.4mg pen-click option sitting between 1.0 and 2.0 is worth knowing about if the dose needs adjusting before committing to the full step up. 8 lbs in the first week at 0.25 is a strong early signal - some people maintain that pace through the lower doses before needing to increase. What does the week-on-week picture look like now?

Started Ozempic mid-June, first four shots at .25, the last couple bumped to .50. Weighing in daily since I'm also doing the Virta Health program - my starting number was 235.4 and I checked in at 217.6 this morning. Sticking to the basics, mostly making sure protein's covered, keeping carbs pretty low too. Only trouble so far has been nausea a handful of days and some constipation. Planning to hold at .50 as long as things keep moving.

Basically held flat, only 2kg gone, through the first eight weeks moving between the two smallest Ozempic doses. Then stepped up to the 1mg mark and shed 38kg across the seven months that followed.

Held at the quarter-mg mark for two to three months, then half a milligram for at least that long too.

Even with weight still dropping and great blood sugar numbers, my doctor insisted I move up to a full milligram since that's the standard protocol.

That turned into three miserable months. Felt queasy fairly often even at the lower amount, but once I hit the full dose the nausea never let up. Some days I could barely make myself eat anything at all. After throwing up a second time (something that almost never happens to me), I decided enough was enough and dropped back to 0.5.

Most people see the nausea fade with time. If it doesn't for you, dropping back to 0.5 and then increasing extremely slowly, maybe just a few clicks weekly, could help.

I'm in my 60s, spent my entire adult life overweight until Ozempic. I understand the diet and exercise fundamentals, I just could never actually stick to them. Even now with much less weight, that struggle hasn't fully gone away. Part laziness, sure. Part is that Ozempic changed what foods actually appeal to me (protein-heavy meals in particular give me trouble). And shedding this much weight has triggered fairly severe orthostatic hypotension, making staying active genuinely difficult. So I already know regain is likely if I stop, and honestly I went off it for a stretch a

Dealing with PCOS, endometriosis, and got an obstructive sleep apnea diagnosis back in May this year. Results ended up pretty similar to what's described here. That said, four separate weeks showed up during the .25 and .50 stretch where the scale and measurements just wouldn't budge. After the fourth .50mg shot, brought up increasing to 1.0mg with the doctor since progress felt inconsistent. Two of those stalled weeks likely lined up with a period, but there's some lingering trauma from before starting ozempic, back when nothing would move the scale at all, and any loss would just bounce back plus extra.

Titrating up made sense given how well 1.0mg has worked since, no stalled weeks at all on that amount.
