I started at a BMI of 35, always hungry with constant food noise. I lost almost 70 lbs but had to stop when my insurance stopped paying. I was just above a 'normal' BMI. I've actually lost another 12 lbs since then, losing at 4 lb/month.
I eat differently now, because I knew the only way to not regain was to simplify my diet. I eat treats maybe once a month, which I used to enjoy weekly. I use very little fat when cooking and eat whole grains with veggies and lean protein. I eat until I feel full, which is probably more than most people need, but the food is healthy. I'm always monitoring what I eat to prevent myself from overdoing it. There is room for twice as much as I eat every day, I'm hungry until mealtime.
I don't know if this will work long term, but I hope so. Both parents are obese, and my dad has a binge eating disorder, I feel destined to struggle.
